<p>St. Clare Commons has an exciting opportunity for an <strong>MDS Nurse</strong> to join our leadership team that is dedicated to fostering a culture of compassion, inclusion, integrity, excellence, and collaboration in alignment with our Mission, Vision, and Values.</p><p>The MDS Coordinator ensures the accuracy of our resident assessments and care plan optimization. To achieve this goal, this position’s responsibilities include, but are not limited to:</p><ul><li>Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines</li><li>Ensuring accurate and timely completion of the MDS process</li><li>Collaborating with the interdisciplinary team to develop individualized resident care plans</li><li>Monitoring and documenting resident progress and responses to care plans</li><li>Participating in the Quality Assurance and Performance Improvement (QAPI) process</li><li>Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery</li><li>Staying updated on MDS regulations and guidelines to ensure compliance</li></ul><p><strong>Requirements:</strong></p><ul><li>Current RN (Registered Nurse) license</li><li>RAC-CT (Resident Assessment Coordinator-Certified) certification is required</li><li>Strong understanding of MDS 3.0, RAI process, and care planning</li><li>Excellent organizational, communication, and interpersonal skills</li><li>Ability to work independently and collaboratively within a team</li><li>Commitment to providing high-quality care to residents</li></ul><p>This position is full-time and on-site.</p>
