Rate of Pay: $79,040 - $87,360/yr

Remote work available after completing 1 month on-site training

CNY Family Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to assist with patient triages and administrative duties in our busy practice. After 1 month on-site training (projected), the position would be work from home.
The Triage Nurse Practitioner/Physician Assistant will provide professional assessments, prioritize treatments according to the urgency of need, and initiate medical care to patients of the practice. We are seeking trained professionals that can communicate with our patients and review and close referral documents and communicate (if necessary) back to the patient's Primary Care Physician.
Nurse Practitioner/Physician Assistant job duties include:
Respond to patient requests or medical advice questions from the patient portal.
Respond to patient requests or medical advice questions from patient phone calls (triage) in a timely manner and communicate directly with the patient.
Review and react to various patient documents including but not limited to; surgeries, stress tests, CT scans, MRI results, Mammograms, Immunizations, etc.
Updating of Medications, Problems, Diagnosis, Allergies and history in the Electronic Medical Record from review of documents.
Communication with patients Primary Care Physicians with any significant change in health status.
Must have a valid NYS Physician Assistant (PA) or Nurse Practitioner (NP) license. A minimum of two (2) years' experience working in a family practice or Urgent Care setting and previous experience working with Medent (EMR) is preferred. Must be energetic, have an excellent working knowledge of chronic disease states and possess critical thinking skills.
