Weare seeking a dedicated full-time Family Medicine Nurse Practitioner to joinour value-based care practice in Marietta, GA. In this role, you will have theopportunity to deliver whole-person care by focusing on quality over quantity,meeting with 10 to 15 patients daily for 30- to 60-minute appointments. Youwill be supported by a robust care team, including a health coach, to helppatients achieve their health goals and improve outcomes.

KeyResponsibilities:
- Providecomprehensive, patient-centered primary care for individuals and families.
- Conduct30- to 60-minute appointments focusing on prevention, chronic diseasemanagement, and population health.
- Collaboratewith an interdisciplinary care team, including health coaches, to ensurecoordinated and holistic patient care.
- UtilizeEMR systems (NextGen preferred) to maintain accurate and timely documentationand ensure coding compliance, including Medicare Advantage and HCC.
- Engagein proactive case management and population health strategies to improvepatient outcomes.
- Leverageexperience in team-based care and Google Workspace to enhance efficiency andcommunication.
Requirements:
- ActiveNurse Practitioner Board Certification in Family Medicine, Primary Care, orInternal Medicine.
- Current,valid, and unrestricted medical or ARNP license, along with a valid DEA number.
- Minimumof 3-5 years of primary care experience.
- Atleast 2+ years of experience in case management, focusing on health preventionand population health.
- Proficiencyin EMR systems, with NextGen experience preferred.
- Expertisein coding and documentation, especially with Medicare Advantage and HCC.
- Familiaritywith Google Workspace tools for collaboration and documentation.
