In this role, you will have the opportunity to deliver whole-person care by focusing on quality over quantity, meeting with 10 to 15 patients daily for 30- to 60-minute appointments. You will be supported by a robust care team, including a health coach, to help patients achieve their health goals and improve outcomes.

Key Responsibilities: - Provide comprehensive, patient-centered primary care for individuals and families.
- Conduct 30- to 60-minute appointments focusing on prevention, chronic disease management, and population health.
- Collaborate with an interdisciplinary care team, including health coaches, to ensure coordinated and holistic patient care.
- Utilize EMR systems (NextGen preferred) to maintain accurate and timely documentation and ensure coding compliance, including Medicare Advantage and HCC.
- Engage in proactive case management and population health strategies to improve patient outcomes.
- Leverage experience in team-based care and Google Workspace to enhance efficiency and communication.
Requirements: - Active Nurse Practitioner Board Certification in Family Medicine, Primary Care, or Internal Medicine.
- Current, valid, and unrestricted medical or ARNP license, along with a valid DEA number.
- Minimum of 3-5 years of primary care experience.
- At least 2+ years of experience in case management, focusing on health prevention and population health.
- Proficiency in EMR systems, with NextGen experience preferred.
- Expertise in coding and documentation, especially with Medicare Advantage and HCC.
- Familiarity with Google Workspace tools for collaboration and documentation.
