Care Coordinator 2 (Field Care Manager 2) Humana Healthy Horizons is searching for a dedicated Care Coordinator 2 to help our members achieve optimal wellness by assessing and evaluating their needs. Your role will focus on guiding members and their families toward essential resources for their health and well-being. This position serves members of the new Indiana Medicaid program - Indiana PathWays for Aging (PathWays), which is designed to help older Hoosiers live at home and access necessary services, leading to improved health outcomes. Key Responsibilities: Build and maintain a trusting, long-term relationship with each member to enhance the quality, continuity, and coordination of their care. Coordinate all necessary medical and non-medical services, addressing functional, social, and environmental needs. Collaborate with the Service Coordinator, Transition Coordinator, and other care team members to meet the identified needs of members. Work with all Medicare payers and providers to coordinate benefits for members eligible for both Medicaid and Medicare. Act as the primary contact for the Interdisciplinary Care Team (ICT), ensuring all member needs are met through collaboration with team members and external resources. Required Qualifications: Valid Registered Nurse (RN) license in Indiana without restrictions. Minimum of one year of clinical nursing experience in case management or similar roles. Strong computer skills with proficiency in Microsoft Word, Excel, and Outlook. Exceptional communication and interpersonal skills, with a knack for building rapport with stakeholders. Demonstrated critical thinking, organizational, and problem-solving abilities. Preferred Qualifications: Bilingual skills in English/Spanish or English/Burmese. Experience in nursing home diversion, long-term care, disease management, or case management. Management experience with Home and Community Based Services waivers. Familiarity with Medicare & Medicaid recipients. Experience working with a geriatric population. Background in health promotion, coaching, and wellness initiatives. Knowledge of community health and social service agencies. Additional Information: This role features a combination of remote work and onsite member visits. Location: Must reside in Indiana. Hours: Full-time, 40 hours per week, Monday through Friday from 8:00 AM to 5:00 PM, with the possibility of overtime. Travel: Willingness to travel approximately 70% for member visits is essential. On Call: Availability for occasional telephonic on-call during nights and weekends may be required. Internet Requirements: A minimum internet speed of 25 Mbps download and 10 Mbps upload is required for remote work. Driver's License: Must possess a valid driver's license, personal vehicle insurance with specified limits, and reliable transportation. Tuberculosis Screening: As this role involves patient interaction, TB screening will be necessary. Scheduled Weekly Hours: 40 Pay Range: $71,100 - $97,800 per year, subject to individual qualifications and performance incentives.
