Position Overview: Under the supervision of the Cohort Manager, the Case Manager will play a dynamic role as a trusted advocate for participants, helping them overcome challenges and achieve their goals. Work to identify service levels and prioritize participant needs, ensuring a holistic approach to care and support Develop, implement, and monitor individualized care plans integrating social determinants of health frameworks in case management plans.
