You will provide advanced palliative care to patients in their homes, helping them to maintain the highest possible quality of life while managing complex medical needs. Work autonomously, collaborating closely with the Palliative Medical Director, to deliver exceptional care and support to patients and families throughout treatment. NP Licensure Required - Tennessee DEA Licensure Required - experience in writing prescriptions preferred PRN Schedule - At least 1-2 days/week Key Responsibilities: Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care. Conduct thorough patient assessments, including physical, emotional, and psychosocial evaluations. Design and implement individualized care plans in collaboration with patients and families. Adjust treatment plans effectively based on patient feedback and progress. Prescribe medications and order diagnostic tests according to state law. Maintain precise and timely clinical documentation adhering to professional standards. Engage in clear communication with physicians, caregivers, and healthcare professionals. Be available for evening/weekend call rotation as necessary. Facilitate referrals for additional care services when appropriate. About You: Specialized Knowledge/Skills: Exceptional leadership and strong communication skills. Independent decision-making capacity and self-motivation. Understanding of palliative medicine principles, home health, and hospice care eligibility, as well as Medicare/Medicaid regulations. Familiarity with the interdisciplinary care process. Meets continuing education requirements for Gentiva and licensing boards. Proficient in point-of-care software and technology. Education/Experience: Master of Science in Nursing from an accredited institution. Minimum two years' experience as a Nurse Practitioner. Preference for a background in Family or Adult Medicine. Experience with Part B billing is beneficial. Able to function autonomously in an advanced practice role. Licenses/Certifications/Agreements: Current RN license and Nurse Practitioner certification/license as required by state law. Collaborative agreement with a supervising physician (or willingness to establish). Part B Provider certification. Federal DEA license and state DEA license (if required). ACHPN certification in Hospice and Palliative Care preferred. Valid driver's license and current automobile insurance.
