Join our team as an Advanced Practice Provider, where you will be a vital member of an integrated Care Team, collaborating with Registered Nurses and Social Workers to improve the quality of life for patients facing chronic conditions, including kidney disease. This is a great opportunity for you to deliver personalized, compassionate care right in patients’ homes, while also conducting virtual assessments as needed. Your expertise will help slow the progression of chronic illnesses and provide crucial support in navigating their health challenges. Key Responsibilities: Conduct comprehensive wellness exams, both in-home and virtually. Educate and counsel patients and families about health programs and resources. Maintain accurate and up-to-date patient records and documentation. Coordinate care with primary care providers, specialists, and ancillary services. Deliver evidence-based care that aims to reduce hospitalizations and enhance patient satisfaction. Prescribe medications, order tests, and collaborate with the patient's physician. Engage in care team meetings to promote integrated healthcare solutions. Assess patients’ health status and develop effective treatment plans. Ensure compliance with all documentation and quality care standards. Assist patients in accessing educational resources for better health management. Perform other duties as assigned. Position Requirements: Active and unrestricted Nurse Practitioner or Physician Assistant license. Board certification for Nurse Practitioner or Physician Assistant. Current and unrestricted DEA certificate. Ability to work independently and practice autonomously. Valid driver's license and access to transportation. Proficient with medical instruments and health information systems. Effective communication skills with patients, families, and healthcare partners. Knowledge of federal and state regulations, including HIPAA and OSHA. Minimum of 2 years of direct patient care experience. Experience in managed care, IPA, or health plan settings.
