What You'll Do The Manager of Social Work is a key clinical leader responsible for guiding a team of Licensed Social Workers and Community Health Workers to assist patients facing complex health and social challenges. This position balances effective leadership with strategic program management, supervising daily operations, ensuring compliance with regulations, and driving quality improvements across social work services. You will work closely with clinical and operational leaders to optimize workflows, enhance patient outcomes, and forge a high-performing, patient-focused team. The ideal candidate will possess both clinical expertise in social work and strong people management skills, effectively combining compassionate patient care with business insights. The Day to Day Supervise, coach, and support licensed social workers and community health workers within your region. Monitor team performance metrics and patient satisfaction, conducting audits to identify and implement improvements. Ensure staff training, workflow education, and compliance with licensing requirements. Participate in the hiring, onboarding, and mentoring of new team members. Cultivate an inclusive team environment that promotes participation, professional development, and knowledge sharing. Collaborate closely with the Senior Manager of Social Work and market leaders to enhance workflows and outcomes. Work alongside market leaders to address team-specific workflow challenges and referrals. Support company objectives with a business-oriented approach and financial awareness. Assess new technologies, tools, and processes for potential departmental improvements. Stay updated on trends, policies, and best practices related to chronic kidney disease treatment. Act as the primary contact for handling PHQ-9 escalations and other clinical necessities. Perform other duties as assigned. Qualifications Minimum Qualifications: Master's degree in social work required. State-specific social work licensure for clinical/independent practice. Advanced certification in Social Work, Nephrology, Case Management, or Mental Health. Proof of passing the ASWB Exam. Minimum of 2 years' experience in chronic disease management or critical care settings. Willingness to travel and be present onsite as required. Internet connectivity requirements: Minimum Speeds: 3.8Mbps/3.0Mbps (upload/download). Preferred Qualifications: At least 1 year of experience leading a team. Able to build and maintain relationships with team members and partners. Strong organizational skills and excellent communication abilities. Value-based mindset focused on improving patient outcomes. Capacity to motivate staff and demonstrate leadership by example. Proficient in MS Word, PowerPoint, and Outlook. Proven ability to excel in entrepreneurial organizations and manage high-performing teams.
