Become the Case Manager You Aspire to Be Collaborate with an interdisciplinary team to tailor treatment plans to meet patient requirements and preferences. Coordinate with team members to form comprehensive discharge plans, including contingencies. Engage in planning and executing patient discharge experiences effectively. Oversee patient experience metrics: service quality, timeliness, appropriateness, expectations, and payors. Facilitate weekly team conferences and manage all changes to treatment plans. Complete necessary case management documentation and addendums. Stay updated on regulatory standards, company protocols, and operational procedures. Review and analyze case management reports and implement appropriate actions based on Key Care Indicators. Understand various commercial contract levels, exclusions, and payor requirements. Attend Acute Care Transfer (ACT) meetings to pinpoint trends and work collaboratively on solutions. Conduct meetings with patients and families within the first 24 hours of admission. Assess goals and finalize case management addendums within 48 hours of admission. Educate patients and families about rehabilitation and the Case Manager role; establish a communication plan. Schedule and conduct family conferences as required. Assist patients in promptly securing resources to prevent discharge delays. Ensure regulatory compliance and manage orthotics and prosthetics orders and payments. Make timely referrals and provide documentation to post-discharge care providers. Ensure accurate documentation of discharge and payor-related information within patient records. Engage in the utilization review process, collecting data, identifying trends, and implementing resolution actions. Be available to participate in the case management on-call schedule as necessary. Qualifications License or Certification: Must be qualified to independently perform an assessment within their discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling). Active licensure required for disciplines that mandate it by state law. Must meet eligibility criteria for CCM® or ACM certification at the outset or achieve it within two years of entering the position. CCM® or ACM certification is required or must be obtained within two years in the Case Manager II role. Minimum Qualifications: For Nursing, an Associate Degree in Nursing and RN licensure is required; a BSN is preferred, while a diploma is acceptable only in states where it is the minimal requirement for licensure. For all other eligible licensed or certified healthcare professionals, a minimum of a bachelor's degree is required, with a graduate degree preferred.
