AltaStaff is a staffing agency currently looking for a Health Assessor to work with our Managed Care Client! This position is fully remote, but 5-10% of local travel may be required as needed so candidates must reside in IL Pay Rate : $36.00 - 38.00 hourly Shift : Monday - Friday 8:30am - 5:00pm CST This position will be responsible for making outbound calls to complete initial and annual health risk assessments, building care plans, answering questions for low level case management. Job Responsibilities (but not limited to): Utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, and promote members' decisions related to achieving and maintaining optimal health status. Typically works with members with moderate to complex clinical needs. Conducts standardized Health Risk Appraisals (HRA) and applicable assessments, including the verification of medical history and current health and wellness needs. Develops a problem list and individualized care plan using program protocols based on the members needs and preferences. Conducts telephonic education to promote self-management strategies for all applicable conditions. Required Education/Experience/Licensure: Must have a Bachelor's or Master's Degree in a human services field (or a valid LPN/LVN licensure) Must have 3-5 years in case management, disease management, managed care, or other medical settings Must be comfortable with a high call volume and self-motivated to make outbound calls LCSW, CCM, LPC, LMFT licensure preferred but not required Must have valid driver's license Interpersonal, analytical, organizational and independent decision-making skills.
