The Behavioral Health Field Care Manager is responsible for conducting face-to-face and telephonic assessments with members, serving as their primary point of contact. This role focuses on providing integrated care, ensuring that members receive timely and high-quality services tailored to their unique needs. By employing various strategies and techniques, the Field Care Manager effectively addresses health issues and resolves barriers to care. With a holistic, person-centered approach, the Field Care Manager aims to enhance behavioral health outcomes, close care gaps, and support Virginia's Medicaid population through comprehensive behavioral health care management. Utilize high-quality, evidence-based behavioral health services, including personalized care coordination, crisis intervention, peer support, and strong collaboration with medical and behavioral health providers. Provide comprehensive, integrated support to members facing behavioral health challenges, particularly children, adolescents, and adults with serious mental illness (SMI) and serious emotional disturbance (SED). Engage with members in their communities, meeting them face-to-face whenever possible to establish trust and enable effective care coordination. Conduct all necessary assessments, including the Comprehensive Risk Assessment (CHRA). Coordinate behavioral health and medical services to ensure proper provider engagement and adherence to treatment plans. Enhance members' health literacy while addressing social needs that affect healthcare outcomes and overall well-being. Act as the lead member of the interdisciplinary care team (ICT), overseeing care planning, transitions, and service delivery. Facilitate ICT meetings to promote seamless communication among providers, Service Coordinators, and Care Management Extenders. Conduct biannual and quarterly face-to-face visits to ensure ongoing monitoring and proactive intervention. Work autonomously while knowing when to reach out for support. Collaborate with internal departments, providers, and community organizations to connect members with appropriate services, creating a culturally competent care experience that respects individual preferences and needs. Follow established processes and procedures to ensure compliance with regulatory requirements from the Virginia Department of Medical Assistance Services (DMAS), Center for Medicare and Medicaid Services (CMS), and the National Committee on Quality Assurance (NCQA). Make a Difference with Your Skills Required Qualifications Bachelor's degree in social work, psychology, or a related health or human services field. Virginia licensed LMHP, LPC, or LCSW. A minimum of 2 years of post-degree clinical experience in a behavioral health setting. Experience in case management for complex SMI or SED populations. Reside in Northern or Charlottesville regions of Virginia. Capable of traveling to facilities and homes for face-to-face assessments. Excellent oral and written communication skills, with the ability to quickly build rapport. Ability to work independently within the role and scope of responsibilities. Proficient in using various electronic information applications and software, including electronic medical records. Intermediate to advanced skills in Microsoft Word, Outlook, and Excel. Capacity to work a full-time schedule. Valid driver's license, car insurance, and reliable transportation. A strong passion for improving consumer experiences in healthcare. Preferred Qualifications Case Management Certification (CCM). 3-5 years of experience in in-home assessment and care coordination. Experience with Medicare, Medicaid, and dual-eligible populations. Knowledge of care for high-risk pregnant and post-partum groups with behavioral health needs. Background in Field Case Management. Familiarity with community health and social service agencies and available resources. Prior managed care experience. Bilingual skills in Spanish, Arabic, Vietnamese, or other languages preferred. Additional Information Workstyle: This is a remote position that involves travel. Travel: Up to 25% of the time to Humana Healthy Horizons office in Glen Allen, VA for collaboration and face-to-face meetings, as well as field interactions with staff, providers, members, and their families. Workdays and Hours: Monday - Friday; 8:00am - 5:00pm Eastern Standard Time (EST). Language Assessment Statement: All associates who communicate with members in languages other than English must complete a language proficiency assessment to ensure competency. Internet Requirements: To support effective work from home, employees must have a self-provided internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Wired, cable, or DSL connections are preferred. This position requires a standard full-time schedule of 40 hours per week. Pay Range: The anticipated salary for this full-time role is between $65,000 and $88,600 per year, reflecting your skills, experience, and qualifications. This job is also eligible for a performance-based bonus incentive plan.
