Meadow Lakes is seeking a Certified Home Health Aide (CHHA) to join our team! Meadow Lakes is a Springpoint Senior Living Community located in East Windsor, NJ. We are seeking a part time CHHA to join our team. At Springpoint, our CHHAs are responsible for providing care and assistance to the patients with activities of daily living in accordance with the patients care plan. The CHHA enables patients to stay in their residence by monitoring and recording patient conditions and providing support and companionship under the direction of licensed professional staff. Responsibilities: Observe the patient's general health conditions, progress, or any unusual health issues and maintain daily records to share with family or supervisor. Guides patients in simple prescribed exercises per patient care plan. Assist patients with transferring when necessary, including but not limited to in and out of beds, baths, wheelchairs. Assist with bathing, skin care, grooming, dressing, and undressing. Maintain records of patient care, condition, progress, and problems to report and discuss observations with supervisor. Change bed linens and clean patient's quarters Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert. Provide emotional support as needed. Serve meals to patients according to prescribed diets and patient request. Clean up after meals in the home as needed. Encourage, transport, or accompany patient to recreational activity programs.
