Social Service Assistant - Part-Time Brinton Manor Nursing and Rehab Center | Long-Term Care Brinton Manor Nursing and Rehab Center is seeking an experienced Social Service Assistant to join our Long-Term Care team . This role is ideal for someone who is compassionate, organized, and experienced working with the geriatric population in a long-term care setting. Key Responsibilities Orient residents and their support systems to the nursing center and assist with adjustment to the new environment Complete social histories, MDS assessments, and develop care plans addressing social and emotional needs Participate in resident care conferences with residents, support systems, and interdisciplinary staff Advocate for and promote resident rights Receive, document, and forward resident and support system concerns to appropriate staff for resolution Follow all facility policies, procedures, and regulatory guidelines Perform other related duties as assigned Education & Qualifications High school diploma or GED required Minimum of one (1) year Social Services experience in a long-term care setting (required) Knowledge of social services practices, regulations, and guidelines governing long-term care Experience working with elderly, ill, and/or disabled populations Strong interpersonal, communication, and organizational skills Ability to work independently and as part of a team Patience, professionalism, and a positive, supportive approach Must be in good physical and mental health Physical examination and/or physician documentation of freedom from communicable disease required upon hire Benefits of Working at Brinton Manor Ongoing professional and clinical training Employee awards and recognition programs Supportive and collaborative work environment Apply Today Make a meaningful impact by supporting residents and contributing to quality care outcomes.
