Practice Setting Hospital-based vascular surgery 24-hour call coverage Adult and geriatric patient population Facility Resources 1 standard operating room 1 Siemens hybrid OR 1 Siemens interventional radiology suite In-house hemodialysis access On-call vascular team Thursday Sunday Support staff includes trained team members and two surgical technologists No certified first assist Procedures Required Aneurysm repairs (including abdominal) Arterial and peripheral bypass Vascular access devices and AV shunts Endovascular procedures Femoral-popliteal and aortofemoral bypass Stenting Reconstruction for deep vein occlusions Peripheral artery disease interventions Credentialing Estimated timeframe: 90 days Travel Lodging reimbursed up to a nightly cap Standard/economy travel expectations Submission Requirements Board certified or truly board eligible Active Kentucky license (IMLC accepted) NPDB self-query dated within 30 days Flu vaccination required at credentialing COVID vaccination preferred
