Position Summary A top-rated home health care agency is seeking a dedicated and compassionate Occupational Therapist (OT) to join our clinical team on a part-time or PRN basis. This role offers the opportunity to deliver meaningful, personalized care to patients in their homes, while working closely with a collaborative care team, including RNs, physicians, and other home health professionals. ⸻ Key Responsibilities: • Conduct comprehensive occupational therapy evaluations, diagnostic testing, and skilled interventions in accordance with a medically approved plan of care. • Develop, revise, and implement individualized therapy care plans tailored to patient needs and physician directives. • Educate and advise patients, caregivers, and home health staff on treatment procedures and supportive activities. • Maintain timely coordination of care with interdisciplinary teams. • Adhere to best practices in infection control, including standard precautions. • Ensure accurate and timely completion of electronic documentation using EMR systems. • Communicate effectively with patients, families, and the care team both verbally and in writing. • Deliver high-quality care consistent with OT professional standards and regulatory guidelines. • Actively participate in self-evaluation and agency Quality Assessment and Performance Improvement (QAPI) initiatives. • Contribute to the interdisciplinary care plan development and adjustments as patient needs evolve. Qualifications: • Graduate of an accredited Occupational Therapy (OT) program. • Current Occupational Therapy license in the state of Indiana. • Minimum of 1 year clinical experience; prior home health experience strongly preferred. • Proficiency in computer-based charting and EMR systems. • Strong clinical documentation and time-management skills.
