Special Projects Bilingual Mandarin or Cantonese Housing Navigator / Care Coordinator - Santa Clara Family Health Plan (SCFHP) Pay Rate: $26.00-$29.00/hr mileage reimbursement Position Type: Special Projects - 12-Month W2 Contract (renewal based on performance and project continuation) Schedule: 9:00 AM - 5:30 PM PST, Monday-Friday Model: Field-Forward (multiple in-person visits weekly) Star Nursing Services is hiring Special Projects Housing Navigators/Care Coordinators to support CalAIM Enhanced Care Management (ECM) and Community Supports under Santa Clara Family Health Plan (SCFHP). This role supports members with housing instability, chronic conditions, and complex social needs. Mandarin, Cantonese due to SCFHP demographics. What You'll Do Provide housing navigation, tenancy sustainment, and support services for Community Supports Housing programs Conduct multiple weekly in-person home visits, housing site visits, and community-based assessments Help members locate affordable housing, complete applications, and communicate with landlords Complete assessments, housing plans, goal setting, and ongoing tenancy support Coordinate with ECM clinical teams, county housing agencies, shelters, and community partners Maintain accurate EMR documentation aligned with SCFHP requirements Support audit readiness, findings response, and corrective action plan tasks Provide bilingual, culturally responsive support throughout the housing process Important - Field Expectations • High field volume with multiple in-person visits weekly • Travel throughout Santa Clara County daily • Remote tasks only for documentation and internal communication Required Qualifications Care/Case Management experience required Minimum 1 year in housing navigation, CalAIM ECM/CS, case management, social services, or public health Strong understanding of CalAIM Community Supports and Enhanced Care Management Knowledge of Santa Clara County housing and social service resources Strong communication, documentation, and time management skills Bilingual English (Spanish or Mandarin or Cantonese) strongly preferred 2 character references Vaccine Requirements: Physical (within the last 30 Days) TB Test MMR, Varicella, Hep B or Titers- Vaccination Tdap/Flu (or decline) Covid Fully Vaccinated (or decline) Must be able to pass background/ urine drug screen Transportation Requirements Unrestricted active California driver's license Valid auto insurance Current registration Reliable vehicle Technology Requirements Intermediate skills in: Microsoft SharePoint Excel Word Teams Adobe (PDF editing, forms, signatures) Experience with EMR/EHR systems used for CalAIM documentation Minimum PC Specifications Windows 10 or higher 8GB RAM minimum (16GB recommended) Intel i5 / AMD Ryzen 5 or better 256GB SSD or larger Chromebooks not allowed Screenshot of computer specs required for interview Education AA/BA, or equivalent experience in case management, housing, social services, or public health.
