Key Responsibilities Perform comprehensive health assessments and physical examinations in patients' homes. Diagnose and manage acute and chronic health conditions. Develop and implement individualized care plans. Prescribe medications and treatments within the NP scope of practice. Order and interpret diagnostic tests. Provide patient and family education on health maintenance and disease management. Coordinate care with primary care physicians and other healthcare providers. Maintain accurate and timely documentation of all patient encounters.
