As a member of our team, you will provide assessments, evaluations, and personalized physical therapy treatments in the comfort of clients' homes. Key Responsibilities: Assess and implement client-specific physical therapy needs, including the development of tailored Plan of Care treatments and goals. Collaborate with clients and their families to support recovery processes effectively. Identify and arrange necessary support personnel, supplies, and equipment for optimal care delivery. Conduct regular client visits as outlined in the Plan of Care. Instruct and supervise clients and other healthcare personnel, including Physical Therapy Assistants.
