Join Our Compassionate Team and Make a Difference! We are seeking a dedicated and empathetic Nurse Practitioner specializing in Palliative Care to enhance our multidisciplinary team. In this rewarding role, you will provide advanced palliative care to patients in their homes, helping them achieve the best possible quality of life while navigating complex medical challenges. You will work closely with the Palliative Medical Director and deliver exceptional care, guiding patients and their families through each stage of treatment. Key Responsibilities: Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered care. Perform thorough patient assessments, addressing physical, emotional, and psychosocial health needs. Develop and implement personalized care plans in collaboration with patients and their families. Monitor and adjust treatment plans to meet evolving patient needs. Prescribe medications and order diagnostic tests in line with state regulations. Maintain accurate clinical documentation that adheres to professional standards. Communicate effectively with physicians, caregivers, and other healthcare team members. Participate in evening and weekend on-call rotation as required. Facilitate referrals for additional care services as necessary. About You: Specialized Skills and Knowledge: Strong leadership and interpersonal skills, along with excellent written and verbal communication abilities. Ability to make independent decisions and work autonomously. Knowledgeable about palliative medicine principles, home health services, hospice care eligibility, and Medicare/Medicaid regulations. Familiarity with the interdisciplinary care model. Compliance with mandatory continuing education requirements for both our organization and state licensing boards. Proficient in using point-of-care software and technology. Education and Experience: Master of Science in Nursing from an accredited institution (or equivalent as per state law). A minimum of two years of experience as a Nurse Practitioner. Preferred specialization in Family or Adult Medicine. Experience with Part B billing is a plus. Ability to work independently in an advanced practice setting. Licenses and Certifications: Current RN license and Nurse Practitioner certification as required by state law. A collaborative agreement with a supervising physician (or willingness to establish one). Part B Provider certification. Federal DEA license and state DEA license (if applicable). ACHPN certification in Hospice and Palliative Care is preferred. Valid driver's license and current automobile insurance.
