Position Summary The Hospital at Home Physician provides acute-level inpatient care to medically appropriate patients in their homes through a hybrid care model that includes in-home visits by nursing staff, virtual rounding, and interdisciplinary coordination. This physician will lead care delivery in a value-based, patient-centered model designed to reduce inpatient utilization while maintaining high clinical quality and safety standards. This role combines traditional hospitalist-level acuity with innovative care delivery, leveraging remote monitoring, mobile diagnostics, and interdisciplinary home-based services.
