Details Home health setting serving West Portland, OR and surrounding areas Day shift, 5x8-hour schedule (8:00–16:30); 4x10 and 3x12 schedules are not available 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points standard No weekend or on-call requirements Charting via Epic Remote Client (laptop) and Rover mobile app; OASIS documentation required Travelers are expected to float between all facilities within the state Requirements Active Oregon OT license required (license required if applicable) Minimum 1 year of skilled home health experience Current BLS certification required Epic EMR experience required OASIS experience required Reliable personal transportation required At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting SSN and DOB required for consideration COVID-19 primary vaccine series plus one booster required Additional Information First-time travelers are welcome to apply Local candidates are accepted; candidates within a 50–75 mile radius of the facility are considered local and are eligible at the same rate Former permanent staff must wait at least 3 months before returning as a traveler and must reside more than 50 miles from the facility Patient caseload is geographically grouped to minimize drive and windshield time; occasional maximum drive radius of roughly 60 miles (60–90 minutes total) per day Business casual attire required; scrubs are permitted but no specific color is required — jeans are not allowed RTO requests should be communicated upfront; minimal RTO is preferred and post-offer requests may be denied American Traveler Job ID P-681557.
