For this role, you need to be a Hospice Nurse who sees the value of Hospice care. You will be providing support, with the goal to help patients live their final days with joy, dignity and comfort. Our Hospice Nurses will conduct patient assessments and education in patient's home, including appropriate supervisory visits for clinical observation of team members. As the hospice nurse, you provide coordination of care through regular interdisciplinary group meetings; developing a team plan of care that supports the patient and family's hopes, dreams and goals before and when dying.
