Nurse Practitioner - Primary Care physician employment in New York : We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in several counties. This position supports the Independent Practitioner Panel (IPP) and involves a combination of remote and in-field duties. About the Opportunity: Hybrid or Remote: Hybrid Shift: Variable Schedule: Weekdays and weekends Hours: Minimum 9 hours/week Setting: Community-based Responsibilities: The Nurse Practitioner (NP) or Physician Assistant (PA) will be responsible for: Reviewing and recommending on consumer assessments Maintaining knowledge of regulatory changes and clinical quality Validating assessment outcomes using clinical skills Collaborating on quality assurance and improvement initiatives Supporting program goals through education and communication Qualifications: 2 years of experience as an APP Master's Degree (NP or PA Program) Active New York State License Active Medicaid Number BLS Certification Computer literacy and tech navigation Experience with telehealth assessments Strong written and oral communication Interpersonal skills and adaptability Knowledge of long-term care programs (preferred) Desired Skills: Experience with adult/geriatric populations Master's Degree (NP or PA Program) BLS Certification Knowledge of long-term care programs Strong interpersonal skills Board Certified or Board Eligible Medical Doctor with or w/o US residency with current US work visa.
