The Mental Health Counselor is responsible for providing psychosocial assessments, collaborative treatment planning, engagement, referrals, and evidenced-based clinical interventions as indicated for individuals dealing with a variety of mental health and co-occurring substance use issues. As part of the Essential Function for this role, the Mental Health Counselor: Conducts initial psychosocial assessments and provides initial treatment recommendations, and collaboratively with the patient develops a personalized treatment plan with emphasis on evidence-based practice within a harm reduction orientation Provides resources and referrals as indicated to facilitate achievement of a patient’s treatment goals Provides psychotherapy and crisis intervention to individuals and families who are dealing with a variety of issues, including but not limited to mental health, co-occurring substance use disorders, chronic medical conditions, including HIV, HCV, and issues related to sexual orientation and gender identity Adheres to program expectations and requirements surrounding productivity, documentation timelines and standards, and overall quality of patient care Works within an integrated treatment model, coordinating treatment with both internal and external providers as indicated Qualified Candidate will have a Masters of Social Work or other related field with two (2) years of related experience; LMSW or LMHC required, LCSW preferred; candidate must be able to communicate clearly and professionally in writing and verbally, in English (with additional Spanish-speaking strongly preferred); demonstrate familiarity with computers and MS Office software (Outlook, Word) and effective time management and organization skills; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential. Job Type: Full time, 40 hours (hybrid/flexible) Required Education: Master’s degree in social work (or similar); LMSW or LMHC required, LCSW preferred Required Experience: Two (2) years of relevant experience; knowledge of Diagnostic and Statistical Manual of Mental Disorders; working knowledge utilizing electronic medical records software (Medent) strongly preferred. The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.
