Patient Care Responsibilities Provide direct nursing care including medication administration and management of ventilators, feeding tubes, tracheostomies, and other medical devices Conduct supervisory visits every two weeks (plus monthly or every 60 days) in patients’ homes Monitor and assess the child’s condition and respond to changes in health status Perform and document clinical assessments including vital signs, pain management, and overall well-being Follow and implement the physician-prescribed care plan Coordination of Care Collaborate with physicians, healthcare professionals, and other nurses to ensure comprehensive care Communicate changes in the child’s condition promptly to the healthcare team Maintain accurate documentation including progress notes, medication records, and patient reports General Responsibilities Provide supervisory support to parent caregivers Maintain accurate and detailed reports and patient records Administer medications and monitor for reactions or side effects Record patient medical information and vital signs Monitor and report symptoms or changes in condition Adjust treatment plans based on patient responses and clinical assessment Assist with examinations and treatments when necessary Qualifications Education Licensed Practical Nurse (LPN) accepted Requirements Active LPN License in the State of Georgia CPR Certification (Required) 1–2 years of related experience or equivalent training Strong communication skills and professional demeanor Reliable transportation and valid driver’s license Job Details Job Type: Part-time / Contract Pay: LPN: $40/hour
