The Licensed Clinical Social Worker will work as part of the Mobile Outreach Safety Team (MOST). MOST serves as a vital, on-the-ground response to individuals experiencing mental health crises. Primary Objectives Provides professional-level clinical social work services, including counseling, psychotherapy, consultation, and outreach, to individuals, families, and groups across residential, home-based, inpatient, outpatient, rehabilitation, correctional, and similar settings. Conducts comprehensive mental health assessments and diagnoses mental, emotional, and behavioral conditions; develops and implements treatment plans; and delivers therapeutic interventions that promote healthy coping strategies, stabilization, and overall well-being. Applies established methods, principles, and techniques of clinical social work including: casework, group work, community organization, administration, planning, consultation, research, and psychotherapy to address mental and emotional conditions, illnesses and disorders, family system challenges, and developmental needs. Works collaboratively with public safety and community partners to provide behavioral health services in field-based and community settings, including crisis response, welfare checks, home visits, substance use interventions, individual assessments, and support for the development and implementation of mental health programs; performing related tasks as apparent or assigned. ESSENTIAL FUNCTIONS Conducts comprehensive biopsychosocial assessments through clinical interviews; reviews medical, social, developmental, substance use, and historical records and collateral information to determine diagnoses (as applicable), strengths, presenting concerns, co-occurring mental health and substance use conditions, treatment needs, resource requirements, and eligibility for programs and services. Provides crisis intervention services using appropriate clinical, behavioral, and environmental controls for individuals experiencing acute mental health crises and/or substance-related impairment, withdrawal, or intoxication; follows established protocols to ensure safety of the individual, staff, family members, and the public; completes required documentation and makes clinical recommendations for voluntary or involuntary hospitalization or higher levels of care when indicated. Participates in treatment planning and implementation by attending treatment team meetings, case conferences, or individual sessions; reports clinical observations regarding progress and recommends modifications to treatment plans as appropriate; evaluates functional and daily living skills and coordinates interventions addressing mental health, substance use, and co-occurring treatment needs including educational and vocational services. Provides individual, family and group therapy or clinical interventions using evidence-based therapeutic techniques appropriate to mental health and substance use disorders, including relapse prevention, harm reduction, motivational approaches and trauma informed care; determines appropriate methods, intensity, and duration of services and adjusts interventions based on clinical progress and outcomes. Develops, coordinates, and contributes to discharge and aftercare planning; educates individuals regarding available mental health and substance use treatment resources; facilitates transition to the least restrictive and most appropriate level of care; advocates for clients through coordination with courts, schools, social services, justice partners, and community organizations. Directs and coordinates case management services to ensure treatment plans addressing behavioral health and substance use needs are implemented, services are integrated, and client progress is monitored and evaluated. Maintains accurate, timely, and compliant clinical documentation, including progress notes, assessments, reports, billing records, and statistical data related to mental health and substance use services, in accordance with applicable laws, regulations, and organizational policies. Reviews and evaluates clinical documentation prepared by professionals, paraprofessionals, and technical staff to ensure treatment effectiveness, service quality, regulatory compliance, and adherence to documentation standards; provides recommendations for improvement as needed. Evaluates program services and activities to ensure integrity, effectiveness, and compliance with agency policies, accreditation standards, licensing requirements, and community standards of care. Collaborates with public safety, emergency response, and community partners to support client stabilization, safety, and continuity of care; provides field-based and community-based behavioral health services as required., including linkage to detoxification, outpatient, residential or recovery-oriented substance use services, as appropriate. Applies clinical expertise to provide consultation, assessment, and evidence-based recommendations related to mental health crises, substance use, co-occurring disorders and crisis de-escalation in public safety and community response contexts. Provides clinical expertise and consultation as a subject matter expert; may testify in court or administrative hearings as required. Participates in community outreach, interagency coordination, and advisory activities; serves on boards, committees, or workgroups related to behavioral health, crisis response, and community services; and acts as liaison to agencies providing mental health services. Develops and delivers training and presentations on behavioral health topics, crisis intervention principles, and best practices for professionals, community partners and service providers. Keeps current on emerging trends, evidence-based practices, and innovations in crisis response models. Provides oversight, supervision, and training as a field instructor for graduate-level social work students as assigned. May accompany public safety or community response partners as a non-driving passenger during routine and emergency responses to provide on-scene clinical assessment and support. Develops and implements integrated treatment plans that address both mental health and substance use disorders, including opioid use disorders, concurrently, promoting holistic care and recovery. Maintains professional and ethical boundaries in all work-related interactions by adhering to organizational policies, respecting confidentiality, and exercising sound judgment when engaging with clients, colleagues, and the public.
