We are seeking an experienced Aged Care Coordinator to join our dynamic and collaborative team.
The successful candidate must be a strong team player, have exceptional organisational skills, a passion for working with older people, and a desire to provide exceptional customer service. Effective communication skills, including strong listening skills, are essential.
This is a busy role and would ideally suit someone experienced in a similar position. Experience in the Community Aged Care Industry is highly regarded, particularly with knowledge and experience in delivering Government Funded Home Care Packages.
This role will focus on case management, customer service, and administrative functions.
The Position Includes:
- Maintaining client files and compliance to meet organisational and quality and safety commission standards
- Conducting a range of client assessments and documentation
- Case management of clients' Home Care Packages
- General administration duties - monitoring and replying to emails, data entry, scanning, and filing
- Providing excellent customer service to clients, staff, and stakeholders
- Managing clients' budgets in line with allocated government funding
- Teamwork, being able to work collaboratively toward a common goal whilst working independently
- Quality assurance
Working Hours:
This Position is a Monday to Friday role, predominately 8.30am - 4.30pm.
Job Type: Permanent Part-time, 37.5hrs/Week.
Capability to work on-call on rotation.

About You:
- Minimum 2+ years of experience in ageing care or community care and understanding of home care packages (preferably)
- Excellent communication skills, both verbal and written
- Compassion and enthusiasm, with a friendly approach and positive demeanour
- Ability to provide documentation for your right to work in Australia
- Passion for Aged Care
- Able to commence work as soon as practicable
Qualifications
Minimum requirement of Certificate III in a care relevant qualification including Aged Care, Disability, Care Planning, Individual Support, Support Work.
Australian Driver's Licence & Reliable Vehicle with Comprehensive Insurance
Current National Criminal History Check
Current First Aid and CPR Certificate
Blue Card - Or Willing To Obtain
Preferred Skills
- Minimum 1 year experience within a similar role is highly regarded
- Experienced with government funded home care packages
- High proficiency with the Microsoft Suite (including Word and Excel) and confident with computer and tablet use
- Confident, competent, and a positive, forward-thinking can-do attitude
- Demonstrated ability to effectively time manage, plan, and prioritise a high volume of tasks independently
- High attention to detail is necessary
- Excellent verbal & written communication, interpersonal, and rapport-building skills
