Summary:
We are seeking a dedicated and organizedReferral Coordinator to join our team. In this role, you will be responsible for managing patient referrals, ensuring that both our patients and primary care physicians (PCPs) are informed and engaged in the referral process. This position requires a proactive approach in initiating the referral process, maintaining accurate patient records, and ensuring the timely receipt of necessary referrals.

Which Electronic Medical Record (EMR)/Electronic Health Record (EHR) systems have you utilized during your professional experience?
Responsibilities:
· Initiate the referral process by contacting the PCP and informing them about the mutual patient requiring a referral.
· Manage high volumes of phone calls daily, which includes outbound calls to PCPs and patients.
· Inform patients about their upcoming appointments and the need for a referral from their PCP. Encourage patients to follow up with their PCP to ensure the receipt of the referral.
· Regularly review patient charts and update records with the latest referral forms. If existing referrals have expired, coordinate with the necessary parties to obtain updated ones.
· Organize and categorize data using Excel, sorting by insurance types, and identifying patients requiring a referral.
· Monitor and track the referral process to prevent any dropped balls. Ensure the referral is received before the patient's scheduled appointment or reschedule as necessary.
· Maintain open communication lines with patients and PCPs to confirm receipt of referrals.
· Maintain a high level of organization and accuracy to stay ahead of appointment schedules.
Qualifications:
· At least 1 year experience in a similar role
· Familiarity with electronic medical record (EMR) systems, particularly eCW, is a strong plus.
· Proficiency in Excel and data management is essential.
· Comfortable with making a lot of phone calls on a daily basis.
· Strong organizational skills and attention to detail.
· Excellent communication skills, both verbal and written.
· Ability to work in a high-volume, fast-paced environment.
· Ability to maintain patient confidentiality and adhere to HIPAA regulations.
· Ability to take initiative and work independently.
· Strong problem-solving skills and the ability to prioritize tasks.
· Good interpersonal skills, with the ability to communicate effectively with patients, PCPs, and the team.
· Patience and empathy to handle patient concerns and queries.
