Qualifications: - Bachelor’s degree; Registered Nurse. Nursing Board of California. - · Full scope RN license
· Experience in working with patients with substance use disorders
· Primary care experience preferred
· Ability to work as a member of a team
· Ability to work well under pressure and meet multiple and sometimes conflicting deadlines
· Ability to interact in an appropriate and professional manner
· Demonstrated cooperative behavior and positive-problem solving and conflict resolution skills
· Available to work during normal primary care clinical hours—Monday-Friday 0800-1700h
Basic Duties and Responsibilities:
· Conduct comprehensive nursing assessment, including social, medical, mental health, substance use, and medication history. __
· Provide ongoing care management and chronic disease management, education, and support for patients. __
· Work with the PCP to manage substance use disorder (SUD) medications and facilitate the contingency management program for intervention patient participants with stimulant use disorder (StUD) __
· Complete timely and appropriate documentation and record keeping in the electronic health record __
· Population management, including proactive outreach, use of a clinical registry, and regularly reviewing cases with the addiction specialist (a consultant who supports the NCM and PCP in formulating treatment plans and adapting them to optimize treatment response and meet the needs of individual patients) __
· Provide intervention patient participants with health education and counseling on risk reduction, overdose prevention, and self-management skills __
· Maintain a professional environment in a multi-provider, multidisciplinary organization. __
· Provision of care and advocacy for patients of all ages and their families based on prudent nursing judgment and acceptable standards of practice. __
· Perform tasks and procedures within the scope of a registered nurse (R.N.) and legal guidelines of the Nurse Practice Act. __
· Involve patients in the development of the treatment plan, and working with the patients' needs __
· Ability to practice trauma-informed care and apply de-escalation techniques __
· Maintain patient confidentiality in alignment with HIPAA __
· Return phone calls, emails and other forms of communication in regards to trial and routing questions to appropriate prescriber as needed.
