Pharmacist - Inpatient
Doylestown, PA

MUST:
Experienced Pharmacist
Must be licensed as a Pharmacist in Pennsylvania
Must hold a Bachelor s Degree in Pharmacy from an accredited school
Must have inpatient and/or behavioral health
Must have experience ordering lab work
Must have knowledge of the Joint Commission standards

DUTIES:
Pharmacist will be responsible for, but not limited to the following:
Attending patient rounds in the morning
Providing support and knowledge to the team on psychotropic medications and any medical medications they may be on
Therapeutic drug monitoring and metabolic monitoring
Liaison with LTC pharmacy Polaris: problem solving any issues that arise to ensure consistent and timely delivery and access to medications
Auditing Data, Chair-person for P&T, Pain Management Stewardship, sitting in on MEC, Antimicrobial Stewardship, etc.
Supporting the Joint Commission Preparation: Familiar the Joint Commission with standards, performing monthly med room inspections.
