Work Dates Needed : ASAP with 2 weeks credentials for 3 to 6 months. Preferred Schedule : M-F 8am to 5pm with 1:30 days call shift in Freemont or San Jose offices. Weekend day shift call is available to pick up. Worksite Setting : 100% outpatient FQHC. Scope of Work : 18-20 patients a day. Provider will have 1-2 dedicated medical assistants, lab work is off site, family practice sees all ages. Licenses, Certifications, Requirements : Board Certification in FM or IM is a must, BLS, CA state license & DEA. EMR : Epic & Dragon software Competitive pay includes malpractice insurance and travel expenses.
