Behavioral Health Outreach Specialist This position allows for virtual work full-time, with the exception of required in-person training sessions, enabling flexibility and autonomy. The Behavioral Health Outreach Specialist plays a crucial role in ensuring that member treatment plans are accurately followed for less complex cases and actively seeks ways to enhance member health while meeting quality goals. Key Responsibilities: Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in behavioral health case management programs. Assess member compliance with medical treatment plans via telephone or on-site visits. Identify barriers to plan compliance and facilitate resolutions. Recommend treatment plan modifications and coordinate additional services with case management and providers. Identify opportunities that impact quality goals and suggest process improvements. Establish relationships with local, state, or federally funded programs for member referrals. Coach members on techniques to reduce health risks. Prepare documentation and reports on case and compliance updates. Maintain relationships with designated agencies according to appropriate contracts. Minimum Requirements: A high school diploma and a minimum of 1 year of related experience; or a combination of education and experience providing an equivalent background. Preferred Skills and Experience: Experience working with behavioral health and/or substance use disorder populations. Community Health Worker or Case Management experience is preferred. Call center (outbound/inbound) experience is preferred. A BS/BA degree in a related field is advantageous. Bilingual candidates are encouraged to apply. The salary range for this position is $20.23/hr to $34.69/hr based on location. This is available to candidates in California, Colorado, Illinois, Maryland, Minnesota, Nevada, Columbus, OH, and Washington State.
