In the role of Health Care Management Specialist, you will be a vital part of our team, working alongside Registered Nurses and Social Workers in care management. Your key responsibilities will include: Leveraging your knowledge of social determinants of health (SDOH) to identify, research, document, and coordinate essential resources and services that enhance member health and well-being. Guiding members and their families toward suitable care and wellness resources. Serving as a dependable source of information on community services for both members and colleagues at Humana. Providing education, contact details, and support to access critical resources like transportation, food assistance, housing, and medical bill support. Collaborating effectively with Humana team members as part of the Care Management Support team. Demonstrating a commitment to enhancing consumer experiences and positively contributing to our organization's mission. Completing assessments when required. Utilize Your Skills and Make a Meaningful Impact! Required Qualifications: Master's degree in Social Work (Non-licensed MSW acceptable). At least 2 years of experience in a medical or community-based setting. Proficient in Microsoft Office Word and Outlook. Ability to quickly learn new systems and navigate multiple platforms. Experience working with individuals facing challenges related to social determinants of health. Exceptional written and verbal communication skills, able to build rapport with members and stakeholders over the phone. Strong interpersonal skills with a proven ability to manage multiple tasks in a dynamic environment. Respect and understanding of diverse cultures and demographics. Preferred Qualifications: Experience in connecting community resources for healthcare and basic needs. Background in working with members/patients managing medical and mental health conditions. Experience advocating for managed care members across various levels of care. Non-clinical Case Management experience. Fluency in Spanish and Creole. Experience in hospital discharge planning, home health, or hospice care is a plus. Work-from-Home Requirements: Reliable internet service with a minimum download speed of 25 Mbps and upload speed of 10 Mbps; wired cable or DSL is recommended. Satellite, cellular, and microwave connections may only be used with prior leadership approval. Hybrid associates will receive bi-weekly payments to assist with internet costs. Humana will provide necessary telephone equipment to meet job requirements. Work from a dedicated space free of interruptions to ensure protection of member PHI / HIPAA information. Additional Information: Training Hours: Monday-Friday 8:00am-4:30pm (for 2 weeks) Actual Work Schedule: Monday-Friday 9:30am-6:00pm EST Occasional travel for visits to Humana offices for training or meetings may be necessary.
