We are seeking Case Managers to work in our Medication Assisted Treatment (MAT) program in Valdosta, Georgia. The MAT Case Manager/ Certified Peer Specialist (CPS), under general supervision, performs a wide range of tasks in the community outside four walls to assist consumers in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions, peer support interventions provides skills training, education and case management to consumers and their families. Support and facilitate consumer progress towards treatment goals. Extensive documentation is required; accurate and timely charts services and consumer progress that supports care coordination. The MAT Case Manager must be able to work well with community agencies and function as part of a team. As a Certified Peer Specialist (CPS), this position also serves as a consumer advocate; provides consumer information and peer support to consumers in an emergency, outpatient, or inpatient setting. The MAT Case Manager/ CPS Performs a wide range of tasks to assist consumers in regaining control over their own lives and over their recovery process. Function as a role model of competency in recovery and ongoing coping skills.
