Summary In this role, the APP will collaborate closely with the interdisciplinary hospice team to deliver high-quality, patient-centered end-of-life care. Responsibilities include providing direct patient care through comprehensive assessments, expert symptom management, and individualized treatment planning, while also serving as a clinical resource for hospice staff and external care teams to support continuity and excellence in care. Schedule: Part Time .6 FTE | 48 hours every two weeks | Onsite Travel throughout CentraCare Hospice service area *Days| 8-hour shifts | 7:30am – 5pm On-Call every 4th or 5th week/weekend Pay and Benefits: Salary range: $109,990.40 - $139,880 per year Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.
