Telephonic Case Management
- Engage with patients or members via phone to discuss their cases, providing support and information as needed.
- Evaluate patient needs through comprehensive assessments to determine appropriate services or resources.
- Communicate with healthcare providers, social services, and other stakeholders to ensure patients receive necessary support.
- Maintain accurate and detailed records of client interactions, case progress, and outcomes in the case management system.
Qualifications:
- Active PHRN License
- At least 6 months of relevant experience
- Willingness to work onsite in Taguig
